Support for clients transitioning from hospital or long-term care with complex medical, cognitive, or functional needs.
Small-group therapy and one-on-one caregiver coaching to extend support beyond the assessment room.
From walkers and rollators to power and manual wheelchairs, we help clients find equipment that fits their body, their home, and the way they want to move through their day. We support the full pathway — assessment, trials, prescription, funding applications, and follow-up.

Activities of Daily Living (ADLs) are the everyday tasks that make independence possible — dressing, bathing, grooming, toileting, meal preparation, medication management, and moving safely through the home. An ADL assessment looks closely at how a client actually performs these tasks in their own environment, where challenges from injury, illness, aging, or a new diagnosis most often show up.
Our occupational therapist observes real tasks, identifies barriers, and works with clients and families on practical strategies, adaptive equipment, and routines that build confidence and reduce caregiver strain. Findings are summarized in a clear written report that can guide care planning, funding requests, discharge transitions, or ongoing therapy goals.
Every client begins with an initial assessment so we can understand your goals and recommend the right path forward. Direct billing is available for most third-party insurance plans in Canada.
