Adaptive Care Solutions is currently accepting referrals for any occupational therapy assessment. Whether you're a patient, family member, physician, case manager, or community partner — we'd be glad to help.
For patients
Book your first appointment directly through our Jane App. We'll confirm by email and reach out ahead of your assessment with anything you'll need to prepare.
For referring providers
Download our fillable PDF referral form, complete it on your device (including a digital signature), then send the completed form to our lead occupational therapist. Prefer phone or email? We're happy to take referrals that way too.
- 1. Download the fillable PDF and open it in Adobe Acrobat Reader or Apple Preview.
- 2. Complete the fields and sign the signature box digitally, then save.
- 3. Click Send Completed Form and attach the saved PDF to the email that opens.
What we accept referrals for
Any occupational therapy assessment — including but not limited to:
- Functional Capacity Assessments
- Activities of Daily Living (ADL)
- Cognitive Assessments
- Home Safety Assessments
- Seating & Mobility
- Worksite & Ergonomic Assessments
- Post-discharge / Transition of Care
- Falls Prevention
- Caregiver Support & Education
